Quality Continuum HospiceSocial workers are trained counselors who help find resources to meet the needs of each client. The dietitian can help with diets, and ideas or help clients meet their nutritional needs. Hospital beds, shower chairs, and other items are provided as needed. Some medications and dressings are provided, which brings financial relief to patients and their families. Some personal care items are also available, as needed. The philosophy of Hospice is palliative care. It is a unique style of care emphasizing comfort and pain management in what is often a difficult time. Many people would prefer home care to institutional care, and this is where Quality Continuum shines. They help in your home where family is available to be primary caregivers. If this is not possible, they offer their services in the patient’s choice of assisted living or long term care facility. For more information contact: |
